    Step 1: Have screenshot, video, or multiple witnesses to your accusation.
    Step 2: Provide the name of the person you wish to ban.
    Step 3: Give the reason for which they should be banned.

    Essentially it should look like this.

    <Insert screenshot>
    Name: Wodan IronFist
    Reason: He KOS'd me more then once and stole all of my gear.
